  • This booking form allows you (the lead attendee) to book for upto three attendees. If you need to book for more than three attendees please submit a second booking form.

    When submitting a second booking form complete the lead attendee name but do not complete the rest of the section for the lead attendee. Complete the rest of the form for the additional attendees as for the first form. Please manually calculate the total cost from both forms when paying.
